Matrix Special Needs Services

Matrix AdvoCare Network has been providing services for seniors and people with disabilities since 1987.  The expert Matrix RN who specializes in the care of individuals who have Special Needs Trusts provides a comprehensive assessment of each individual client's needs including consultation with the trustee, client, family, physicians, medical equipment providers, school personnel, therapy providers, caregivers, county workers and other interested parties. She then serves the client as requested to assure all needs are met and all payor sources are identified and utilized.

• A comprehensive medical status and needs assessment.

• Documentation to support medically necessary equipment and/or services.

• Assistance in obtaining eligibility determination for county funds and services (MA, CADI).

• Review of home care services to ensure accurate/appropriate care (PCA, PT).

• Assistance in evaluating home accessibility and adaptations issues.

• Investigation of misappropriation of trust funds.

• Review of medical history.

• Access to appropriate medical providers and specialists.

• A health advocate to support client and family.

• Expert Comprehensive Assessment of needs.

• Detailed written report documenting medical necessity of equipment and services, along with appropriate professional recommendations.

• Knowledgeable nurse advocate to assure client receives public funds to which they are entitled. 

• Assessment and advocacy for adequate home care services.

• Thorough home assessment to assure appropriateness of home improvements/ adaptations.

• Review of medical history to address medical issues and obtain appropriate medical providers and specialists.

• Attendance at medical appointments to advocate for client to assure all questions and needs are addressed.

• Advocacy with billing departments to resolve payment issues.

The Matrix RN professional uses her expertise to act as a resource for the trust administrator on a regular basis when the family requests support from the trust for services.  She determines if the family's request for services could financially be met with public health care benefits or other governmental services and gathers resources to support the family's request of such services to meet the client's ongoing and changing needs.
