How
Does Matrix Team Home Care Work?
Matrix Team Home Care provides care recipients with specially selected
caregivers who are chosen for each client. Each care recipient and
family has the opportunity to interview and approve each caregiver who
becomes part of their care team. All caregivers are Matrix
employees and are carefully screened and fully trained to meet each care
recipients needs. A licensed registered nurse develops a Plan of
Care in conjunction with the care recipient and family, and supervises
the caregivers closely to ensure that services are provided skillfully,
respectfully, and safely.
The Matrix Team Home Care Process

Matrix's licensed registered
nurse (RN) meets with you to identify your specific care needs and
your preferences regarding caregivers.

Matrix identifies and
interviews potential caregivers, conducts background checks and
ensures that individuals have the skills needed to be on your care
team.

You meet potential
caregivers to decide those you want on your caregiving team.
Accepting a caregiver is always your choice.

When your caregiving team is
chosen, the Matrix RN develops a Care Plan with you and instructs
your caregivers in implementing your plan.

The Matrix RN provides
regular medication set-up and physician consultation as needed.

RN provides regular supervision so changes in your condition can be
promptly resolved and care adjusted as your needs change

based on your needs and only with your approval.
An RN is on call 24/7 for urgent
needs
RN services are provided in
visits of about one hour or in visits up to 24 hours per day
LPN visits are provided from 2
to 24 hours daily
Home health aide services are
provided from 4 to 24 hours daily
Homemaking services can be
provided from 4 to 24 hours daily
Other professional services are
provided in visits of about one hour duration
