Karen Lawrence, MSN,
PHN,RN-BC,
CCM President and Owner
Karen earned her Bachelor of Science Degree in Nursing from Assumption
College in Worcester, Massachusetts and her Master of Science Degree in
Nursing from the University of Phoenix, summa cum laude. She is certified
in Gerontological Nursing by the American Nurses Credentialing Center and is
a licensed public health nurse. Karen is
a Certified Case Manager and a member of
the
National Association of Professional Geriatric Care Managers. Karen has
extensive experience working with elder adults in long term care and as an
educator working with long-term care nurses and care providers over the past

years. Karen serves Matrix clients
throughout the metro area and joined
Matrix in September 2002. She
has
served as the Clinical Supervisor since
2004 and as the Vice President of Case Management services for Matrix AdvoCare Network in 2007. Karen has served as the Director of Nursing for
Matrix since March of 2009.
Geraldine Bublitz, BSN, PHN, RN
Geri earned her Bachelor's of Science Degree in Nursing from the Minnesota
State University Mankato. She has over 25 years of nursing experience.
She spent several years working in the hospital setting as an operation room
nurse. She also has extensive experience in home health care and
hospice working as a staff nurse and then a case manager for a variety of
clients of all ages and diagnoses. She has expertise in wound care and
dressing and IV therapies as well as all home care and hospice. Geri
works in the Mankato area and joined Matrix in 2012.
Sandy Egan-McNiff, PHN, MA/Licensed Psychologist
Sandy earned her Bachelor of Science Degree at the College of St. Catherine
in St. Paul, MN and her Masters Degree from Drake University in Des Moines,
Iowa. She is a licensed psychologist as well as a public health nurse.Sandy has extensive experience working with all ages in home care,
long term care and outpatient clinics. Her area of expertise is in mental
health assessment and treatment. She works with clients primarily in the
southeast metro area as well as south-eastern Minnesota. Sandy joined
Matrix in September 2002.
Carol Gibbs, RN
Carol earned her Associates Degree in Nursing at Normandale Community
College in Bloomington, MN. Carol has 31 years of nursing experience
including acute care, long term care, hospice nursing, nursing practice in
medical clinics and most recently in home health care. In addition,
Carol received her certificate for Legal Nurse Consulting in 2002. She
enjoys working with clients in their homes as it provides a more personal
approach to nursing. Carol works with clients primarily in the Saint Paul
and the north and northeast suburbs. Carol joined Matrix in October 2010.
Carol also works as a Matrix oncall nurse.
Jenny Hayden, RN
Jenny earned her Associates Degree in Nursing at Normandale Community
College in Bloomington, MN. She has extensive experience working in the
acute care hospital setting with a variety of populations who have
experienced neurological conditions including strokes, brain tumors,
traumatic brain injuries, and epilepsy. Jenny also has experience working in
skilled nursing and assisted living care environments caring for seniors.
Jenny joined Matrix in 2009. Jenny primarily serves clients in the
southern Minneapolis/ Saint Paul area.
Judy Hostnick, RN, MA
Judy earned her nursing diploma from Oak Park Hospital School of Nursing,
her Bachelor of Science in Health Arts from St. Francis College in Joliet,
Illinois, and her Master of Arts in Pastoral Studies from the University of
St. Thomas, St. Paul Seminary School of Divinity in St. Paul, Minnesota.She has extensive experience working with home care and hospice
clients, and formal training and experience in grief counseling.Judy's sense of humor enables her to establish therapeutic
relationships with even the most resistant clients, and her persistence in
problem-solving helps keep clients in their homes.Judy sees clients in south Minneapolis, and has been with Matrix
since March 1998.
Kathleen Kelso, MAPA, BSN, PHN, RN
Kathy
earned her Bachelor of Science in Nursing Degree from the University of
Iowa. She obtained her Master of Arts in Public Affairs with a focus
on Health Care Policy from the University of Minnesota. She has over
40 years of experience in non-profit management, governmental affairs,
nursing and health care policy. Kathy is listed on the Minnesota State
Court System ADR Rule 114 Neutral Roster as a mediator, specializing in
elder mediation and conflict management. Kathy sees clients in the
Saint Paul area and has been with Matrix since 2012.
Sherri Kulseth, RN, BSN
Sherri earned her Bachelor of Science Degree in Nursing from the University
of Minnesota. Sherri has over 10 years of nursing experience, including
hospital, homecare, hospice and nursing home care. Sherri began working for
Matrix in November 2008 as one of the Matrix On Call RN Care
Managers.
Sherri serves all our clients as needed after hours, weekends and holidays.
Cheryl Mantle, BSN, PHN, RN
Cheryl first earned an Associates Degree in Nursing from Atlantic Union
College in Massachusetts. She then went on to practice nursing in Virginia
and eventually moved to Minnesota. She has resided in Minnesota for 18 years
and earned her Bachelors of Science Degree in nursing at Metropolitan State
University. Cheryl
has a holistic approach when working with clients and their families and has
practiced nursing for more than 20 years.Additionally, she has experience in gastrointestinal care. Cheryl is
a certified massage therapist and has an interest in complimentary
therapies. In her spare time Cheryl enjoys photography.Cheryl has been with Matrix since April of 2010. Cheryl works
primarily with clients in the Minneapolis and surrounding suburbs.
Karen Ploog, BSN,
PHN,
RN-BC
Karen earned her Bachelor of Science Degree in Nursing from the College of
St. Catherine in Saint Paul, MN.
She is certified in Gerontological Nursing by the American Nurses
Credentialing
Center and is a licensed public health nurse.Karen has over 26 years of nursing experience including hospital, medical
surgical, dialysis, and ICU.In
these roles she has gained expertise in case management, family dynamics,
and managing difficult patients.
In her spare time she volunteers as a Meals on Wheels coordinator.Karen has been with Matrix since April 2010. Karen works
primarily with clients in Minneapolis and the western suburbs.
