Care Coaching, also referred to as care consulting, care management or case management, evolved out of the shifts in demographics and health care.  It involves assessing a clients physical, mental, and social needs, then developing an individualized plan of care to meet them, be they small or large, medical or practical.  This includes making referrals to appropriate services, and coordinating, evaluating, and adjusting them as needed.

The “golden years” are aptly named for many people, the season in life when freedom, resources, and the wisdom of experience come together.  Aging can also have a painful side, the signs of which you may be seeing in someone you love.  They can be as fleeting as a trembling hand, as heart-rending as a vacant stare, as upsetting as incontinence, or as debilitating as dementia.

Regardless of the symptoms or their cause, one thing is sure, watching someone you love decline because of aging isn’t easy, especially when quality of life is diminished.

Rest assured, you’re not alone.  More Americans than ever before are living decades after retirement.  This, coupled with changes in the health care system, means that seniors are less likely to have strong connections to primary health providers.  They’re also less likely to have local family members to turn to for support, all of which poses critical questions.

ONLY REGISTERED NURSES PROVIDE MATRIX CARE COACHING

            All Matrix AdvoCare Network care consulting is done by registered nurses who are at the top of their profession.  All have expertise in gerontology, as well as special skills in assessment, care planning, and knowledge of community resources.

            Each helps only a small number of clients, which fosters a genuinely deep and therapeutic relationship that is wholly dedicated to meeting client needs.  A client’s personal dignity and self-esteem are foremost, especially when decisions and choices are considered.

            And, since Care Consultants make recommendations, not decisions, they work cooperatively with health care providers and you to develop and implement an appropriate plan of care.

CARE CONSULTANTS REMAIN INVOLVED FOR AS LONG AS NECESSARY 

            Care consulting can be a one-time fix of immediate needs.  It can also be an ongoing relationship that ensures a client receives the specific care necessary to optimize quality of life, even if he or she is in a nursing home.

            For example, Care Consultants advocate for clients at medical appointments and assure correct administration of medications, critical if more than one physician is writing prescriptions.  They assess and equip living environments for safety.  If help is needed with daily activities from grooming to shopping, they find appropriate services.  Social needs, such as transportation, companionship and spirituality, are addressed.  In other words, Care Consultants do what no single service does.  No problem is too small or too large for them to resolve.

WE INCLUDE YOU EVERY STEP OF THE WAY 

            We know how difficult it can be to have a stranger step in and care for someone you love.  So our care mangers ensure that you’re integrally involved to the degree you’d like.  They’ll discuss concerns you may have, whether it’s finding an adult day health program or the best residential facility – and work through all the medical, social, psychological, and practical issues that are involved.

AN INFORMATIONAL VISIT IS PROVIDED FREE OF CHARGE 

            The extent to which a Care Consultant can enhance quality of life is directly related to how early intervention is sought.  Left unattended, small problems have an uncanny way of becoming bigger.   Earlier intervention is also highly cost-effective, especially when handled by a knowledgeable Care Consultant, and can minimize undue expense.

            So if you think that seemingly minor behavioral or physical changes are merely the “normal” aging process, you may want verification through an assessment from a Care Consultant.

            We’ll arrange a free, informational visit at a place convenient for you to introduce you and your senior to our services and answer your questions.  If a Care Consultant is retained, we’ll move to the next step.

OUR SERVICES INCLUDE:

  • Conducting a Comprehensive Assessment of each client's situation.

  • Developing a personalized Plan of Care listing high quality,
    cost effective care options and costs.

  • Implementing the Plan of Care chosen by the client/family.

  • Adjusting the Plan of Care as needed.

  • Providing advocacy for doctor, dental and hospital appointments.

  • Helping obtain a new physician(s) or second medical opinions.

  • Helping choose a new living environment and arranging a move,
    both within and outside Minnesota.

  • Helping complete Health Care Advance Directives (living will).

  • Helping obtain health equipment and supplies.

  • Providing health teaching and implementing preventive strategies.

  • Discuss the client's situation with you at no charge--on the phone
    or in person--to determine if we can be helpful to you, and if we can't,
    we will refer you to other agencies.

  • If requested, provide references from current or former clients.

  • Provide a written fee schedule and explain how we charge
    for services.
     

IF YOU DECIDE TO USE MATRIX SERVICES, we will:

  • Request a signed Service Agreement prior to beginning service.

  • Ensure the confidentiality of client information at all times.

  • Provide only those services authorized by the client/responsible
    party.

  • Provide regular verbal and written reports documenting our
    findings, recommendations and actions.

  • Foster the client's autonomy, well-being and safety at all times.

  • Consider the client's family an integral part of the client's Plan of
    Care.

 WE WILL NOT:

  • Benefit financially from any referral we make to community services.

  • Provide financial management or fiduciary services.

  • Act as guardians or conservators.

  • Accept gifts or tips from clients.

  • Provide services we are not qualified to provide.
